The aim of this study was to compare phenotype of patients with pituitary, adrenal and ectopic CS and identify the differences regarding biochemical parameters, clinical presentations, and comorbidities in CS patients who were diagnosed at the single endocrinological center in Wroclaw.<br />The study population involved 64 patients with CS (53 women and 11 men) diagnosed in Department of Endocrinology, Diabetes and Isotope Therapy in 2000-2018. Patients were divided into three etiologic groups pituitary dependent-CS (P-CS) (64%), adrenal dependent CS (A-CS) (25%), and CS from an ectopic source (E-CS) (11%).<br />Percentage of men in the A-CS group was significantly higher than in the other etiologic groups. ACTH, UFC, and cortisol in DST were significantly higher in E-CS group compare to P-CS and A-CS (p &lt; 0.05). Mean potassium level in E-CS group was significantly lower than in P-CS and A-CS (p &lt; 0.05). Median of time elapsed to diagnosis was significantly lower in the E-CS group compared with either the P-CS and the A-CS group (p &lt; 0.01). The most frequently symptoms in CS patients were skin alterations (82.8%), weight gain (81.2%), and hypertension (81.2%).<br />The epidemiology of CS is changing toward a growing proportion of A-CS. All patients with E-CS presented a profound hypokalemia. Salient hypokalemia could be a biochemical marker more suggestive for E-CS rather than P-CS. Arm ataxia causes dysmetria (over- or under-shooting of targets) with many corrective movements. As a result, people with cerebellar damage exhibit reaching movements with highly irregular and prolonged movement paths. Cerebellar patients are also impaired in error-based motor learning, which may impede rehabilitation interventions. However, we have recently shown that cerebellar patients can learn a simple reaching task using a binary reinforcement paradigm, in which feedback is based on participants' mean performance. Here, we present a pilot study that examined whether patients with cerebellar damage can use this reinforcement training to learn a more complex motor task-to decrease the path length of their reaches. We compared binary reinforcement training to a control condition of massed practice without reinforcement feedback. In both conditions, participants made target-directed reaches in 3-dimensional space while vision of their movement was occluded. In the reinforcement training condition, reaches with a path length below participants' mean were reinforced with an auditory stimulus at reach endpoint. We found that patients were able to use reinforcement signaling to significantly reduce their reach paths. Massed practice produced no systematic change in patients' reach performance. Overall, our results suggest that binary reinforcement training can improve reaching movements in patients with cerebellar damage and the benefit cannot be attributed solely to repetition or reduced visual control.Vibration interventions are used in neurorehabilitation to improve postural control in recent years. Little is known about the immediate effects of vibration interventions on postural control in patients with ataxia. The aim of this study is to investigate and compare the immediate effects of local vibration (LV) and whole-body vibration (WBV) on postural control in patients with ataxia. This study was designed as cross-over, single blind randomized clinical trial. Twenty-one patients with ataxia met the inclusion criteria. LV (frequency, 80 Hz; amplitude, 1 mm) and WBV (30 Hz, 2 mm) were applied to all patients. There was a 1-week washout time between interventions. Each patient was assessed 3 times pre-intervention and 1 and 60 min post-intervention. The assessor was blinded to the interventions. Outcome measures were limits of stability (LoS), and postural sways (Bertec Balance Check Screener), gait parameters (GAITRite), and static balance (one-leg stance test). Twenty patients completed both interventions. The mean patient age was 39.43 ± 9.67 years. LV increased the left-LoS post-vibration (1 and 60 min post) more than WBV did (p ˂ 0.05). LV increased the LoS stability score and the base of support at 1 min post-vibration, while WBV decreased them (p ˂ 0.05). This study demonstrated different immediate effects of a single session of LV versus WBV and showed that LV has better effects on postural control in patients with ataxia. The 57 years old patient with a history of spondylodiscitis and Staphylococcus aureus-associated endocarditis was admitted to the intensive care unit for acute cerebral infarction. The patient developed signs of infections and the CVC was removed 11 days after placement. The infectious agent was identified by standard diagnostics to the genus level as belonging to the Enterobacter cloacae complex, and additional molecular testing determined the species as E. The catheter was selected for a study aiming to identify the influence of blood components on the formation of central venous catheter-associated biofilms. In this course a massive biofilm was recognized and is presented here.<br />Galactomannan antigen (GM) testing is widely used in the diagnosis of invasive aspergillosis (IA). Digestive enzymes play an important role in enzyme substitution therapy in exocrine pancreatic insufficiency. As digestive enzymes of fungal origin like Nortase contain enzymes from Aspergillus, a false-positive result of the test might be possible because of cross-reacting antigens of the cell wall of the producing fungi. We, therefore, asked whether the administration of fungal enzymes is a relevant cause of false-positive GM antigen test results.<br />Patients with a positive GM antigen test between January 2016 and April 2020 were included in the evaluation and divided into two groups group 1-Nortase-therapy, group 2-no Nortase-therapy. In addition, dissolved Nortase samples were analyzed in vitro for GM and β-1,3-D-glucan. For statistical analysis, the chi-squared and Mann‒Whitney U tests were used.<br />Sixty-five patients were included in this evaluation (30 patients receiving Nortase and 35 patients not receiving Nortase).
