The aqueous-phase reaction of dimethylarsinc acid (DMAs(V)) with NaBH4 (THB) was studied under non-analytical conditions (1000 μg/mL As, 0.1 M HCl, 1% NaBH4) with the aim of identifying intermediates and reaction products. The use of direct analysis in real time (DART) with high-resolution mass spectrometry (HRMS), in combination with two different chemical vapor generation systems, allowed the identification of some species not detected by GC-MS such as Me2As-AsMe-AsMe2 and the arsonium species [Me3As-AsMe2]+ and [Me2As-AsMe2-AsMe2]+. Many other methylated species of arsenic containing up to four arsenic atoms have been observed. Unfortunately, the oxidation mechanism that took place in the DART source interfered with the identification of some of those species formed in solution following THB reduction. The species identified by DART-HRMS, together with those previously identified by GC-MS (Me2AsH, Me2AsOH, Me3As, Me3AsO, Me2AsAsMeH, Me2AsAsMe2, and Me2As-O-AsMe2)' enabled the formulation of hypotheses on the possible reaction pathways and revealed an aqueous-phase reactivity of DMAs(V) which could not be explained on the basis of current knowledge. Graphical Abstract.Squamous cell carcinoma in situ (SCCIS) of the nail unit is a complex malignancy; with little understanding of rate of upstaging or occult invasion in these patients. We sought to evaluate the rate of upstaging in nail unit SCCIS after Mohs micrographic surgery (MMS). Retrospective review of 346 patients who referred for and underwent MMS for biopsy proven SCCIS on the hands and feet between January 1, 2000 and December 30, 2019. Only cases in the nail unit were included. Clinical, surgical details, histopathological features, HPV status, and rate of upstaging were recorded. Thirty-one cases met the inclusion criteria and were analyzed. Twenty-four patients were males (77.4%). The mean age was 55 years (SD 17.26, range 27-84). The mean clinical size was 9.9 mm; 19 cases tested for HPV, 15/19 were positive (78.9%), and 8/19 (42.1%) were associated with high-risk HPV. Three patients (9.7%) were upstaged to invasive on either MMS margins or tumor debulking. Limitations included a relatively small sample size and retrospective in nature. The rate of upstaging of SCCIS in the nail unit is not frequent, and when upstaging occurred it was focal, superficial, and with no PNI or bone invasion.
Fedratinib (INREBIC
), a Janus kinase 2 inhibitor, is approved in the United States to treat patients with myelofibrosis. Fedratinib is not only a substrate of cytochrome P450 (CYP) enzymes, but also exhibits complex auto-inhibition, time-dependent inhibition, or mixed inhibition/induction of CYP enzymes including CYP3A. Therefore, a mechanistic modeling approach was used to characterize pharmacokinetic (PK) properties and assess drug-drug interaction (DDI) potentials for fedratinib under clinical scenarios.
The physiologically based pharmacokinetic (PBPK) model of fedratinib was constructed in Simcyp
(V17R1) by integrating available in vitro and in vivo information and was further parameterized and validated by using clinical PK data.
The validated PBPK model was applied to predict DDIs between fedratinib and CYP modulators or substrates. The model simulations indicated that the fedratinib-as-victim DDI extent in terms of geometric mean area under curve (AUC) at steady state is about twofold or 1.2-red and then re-escalated to 400 mg in a stepwise manner as tolerated after the strong CYP3A4 inhibitor is discontinued.
Young adults are vulnerable to weight gain and dietary behaviours such as 'eating on the run' are likely contributors. The objective of this study was to examine eating and drinking behaviours during transport journeys in a sample of young adults using wearable cameras that take continuous images every 30s.
Seventy-eight 18-30year olds wore an Autographer wearable camera for three consecutive days. Image coding schedules were designed to assess physical activity (included transportation) and diet. For the general description of data, frequency analysis was calculated as image number (percentage) and mean (± SD) or median (IQR) when appropriate.
A total of 281,041 images were coded and 32,529 (14%) of images involved transport. The median (IQR) camera wear time was 8h per day (7-9h). The camera images identified 52 participants (67%) either eating or drinking during transport (excluding water). A total of 143 eating and drinking occasions were identified, averaging 3 occasions per person over the three study days. Fifty five (38%) eating episodes were identified by the camera images of which 27 (49%) were discretionary and 88 (62%) drinking episodes were identified of which (45%) were discretionary.
This study confirms that transport is a potential setting for intervention. Young adults are consuming discretionary food and beverages during transport which may contribute to energy-dense diets and compromise diet quality. Substituting unhealthy with healthy food advertising and potentially prohibiting eating and drinking whilst on public transport is suggested.

To investigate the role of IL-33 in gouty arthritis.
174 Balb/c (wild-type) and 54 ST2
mice were used in this study. In vitro experiments were conducted in bone marrow-derived macrophages (BMDMs). Synovial fluid samples from gouty arthritis (n = 7) and osteoarthritis (n = 8) hospital patients were used to measure IL-33 and sST2 levels.
Gout was induced by injection of monosodium urate (MSU) crystals in the knee joint of mice. Pain was determined using the electronic von Frey and static weight bearing. Neutrophil recruitment was determined by H&E staining, Rosenfeld staining slides, and MPO activity. ELISA was used for cytokine and sST2 measurement. The priming effect of IL-33 was determined in BMDM.
Synovial fluid of gout patients showed higher IL-33 levels and neutrophil counts than osteoarthritis patients. learn more In mice, the absence of ST2 prevented mechanical pain, knee joint edema, neutrophil recruitment to the knee joint, and lowered IL-1β and superoxide anion levels. In macrophages, IL-33 enhanced the release of IL-1β and TNF-α, and BMDMs from ST2
showed reduced levels of these cytokines after stimulus with MSU crystals.
