Evaluation of Metered Serving Inhaler Use Strategy and also Reaction to Educational Education
Symptom control in patients with angina due to coronary artery spasm can be challenging as pharmacological agents are often prescribed on a trial and error basis. With this case, we present a novel approach toward a more individualized pharmacotherapy in coronary artery spasm using a modified protocol for invasive coronary spasm testing, which could avoid ineffective treatment trials in such patients.Cerebellopontine angle (CPA) lipoma is an extremely rare lesion representing only 0.1% of all the CPA tumors. We present a case of a 56-year-old woman with a 6-month history of vertigo. Cerebral MRI showed a left CPA lipoma. The patient was managed conservatively. The case was a 4-day-old boy referred to the hospital with a diagnosis of Pierre Robin Sequence and with airway Obstruction. Using a laryngoscope in the hyperextended position and putting pressure on the cricoid, the vocal cords were visualized, and with the help of an assistant, the intubation was performed.Temporomandibular joint ankyloses (TMJA) may manifest in patients with several predisposing systemic conditions. A case of extraarticular TMJA is presented in a patient diagnosed with fibrodysplasia ossificans progressive (FOP) is presented. The features, diagnosis, and management of TMJA superimposed on this condition are presented through a qualitative systematic review of literature.Novel translucent monolithic zirconia has improved optical properties, and it may fulfill patient's esthetic demands and overcome the chipping risk of bilayer metal-ceramic restorations. New zirconia's microstructures allow us to mimic natural teeth. This report describes the case of an 84-year-old male who was brought to the emergency room because a dental bur was swallowed accidentally during a dental procedure. The foreign body was successfully removed by gastroenterologists endoscopically 8 days after the ingestion and was identified as a 2-cm-long dental bur.
The After Cancer Treatment Transition (ACTT) program at Women's College Hospital (Toronto) is a transitional follow-up program for patients, their families, and healthcare providers to address the broad range of post-cancer treatment and survivorship needs. This publication describes the systematic development and implementation of the ACTT program, with a focus on the advanced practice nursing (APN) role.
ACTT development required the collaboration of an APN, a general practitioner in oncology (GPO), and an inter-professional team. ACTT developers proposed a clinic structure in an ambulatory setting, linking healthcare professionals to provide post-treatment follow-up and ongoing survivorship care. Post-treatment guidelines were developed based on expert oncologist consensus, cancer site group input, and evidence-informed guidelines or best practice recommendations.
Initial challenges and concerns were rooted in the requirements that post-cancer treatment care was maintained and survivor needs were addsition of care, and conducting comprehensive evaluations of patient reported outcomes.Despite the desire of gynecologic oncology (GO) patients to speak openly about their sexual health experience with nurses, nurses often feel ill equipped to engage in these conversations. There are very few educational interventions available to GO nurses to improve sexual health communication with patients. The purpose of this narrative review is to identify and summarize existing educational interventions in this field. A literature search conducted in three databases, for the years 2010 through 2020, identified 11 papers. The results of the review indicate a mix of nurse training modalities and explore the potential for improving this communication. Existing training programs vary in terms of mode of delivery (online or in person), length, type of instructor, learning strategies and themes addressed. Overall, however, the results show a general lack of sexual health training for nurses caring for GO patients. The purpose of this multi-year study was to replicate a successful breast cancer awareness campaign pilot, targeting young women, and evaluate the campaign effectiveness on multiple Canadian post-secondary school sites. A Canadian charity, Team Shan Breast Cancer Awareness for Young Women (Team Shan), facilitated awareness campaigns on college and university campuses across Western Canada from 2010-2016. Using a pre-post design, young women (17-29 years) on 11 targeted campus sites participated in completing pre- (n = 880) or post-campaign (n = 794) evaluation questionnaires. Questions were designed to evaluate awareness campaign activities, key take home messages, and breast cancer knowledge transfer. Team Shan was successful in developing a variety of strategies to inform young women about their risk of developing breast cancer. The campaigns made a positive impact on young women as an effective public health communication initiative. Findings support health promotion strategies to reach young women, a population at risk of developing breast cancer.
Cancer-related cognitive impairments experienced by cancer survivors cause many to seek non-pharmacological intereventions to manage these symptoms. The aim of this systematic review was to evaluate the effects of one such intervention, mindbody exercise (MBE), on cognitive function in cancer survivors.
Searches for relevant studies were conducted in four electronic databases, including PubMed, Embase, Scopus, and Web of Science. The Joanna Briggs Institute and Jadad scales were utilized to evaluate the quality of the selected studies.
Eleven studies including 1,032 participants, published between 2006 and 2019, were selected for review based on specific inclusion criteria. Our results indicated that interventions including, yoga, tai chi, and qigong may improve objective and subjective cognitive function in cancer survivors.
Cancer survivors experiencing cognitive symptoms may benefit from participation in MBE. learn more Adequately powered randomized controlled trials are required to establish the short- and long-term effects of MBE on cognitive functioning.

The growth in prevalence of preoperative anemia in patients undergoing gynecological oncology procedures warrants greater attention to early identification for optimal surgical outcomes. This was a quantitative retrospective observational study consisting of 284 patients undergoing gynecological oncology procedures. The study sought to determine the frequency of anemia, iron deficiency and the effect of anemia on the number of blood transfusions from January 1 to December 31, 2014. Patients with anemia had significantly higher transfusion rates (44% versus 11%, p less then 0.0001), considerably higher number of units transfused per patient (mean 1.19 units versus 0.28 units, p less then 0.0001) and longer length of stays post-operatively (mean 5.9 days versus 4.6 days, p=0.0008). It was concluded that early identification and treatment of anemia is a key opportunity to optimized surgical outcomes.Tocilizumab, a monoclonal antibody drug against interleukin-6 receptor, has been reported to cause wound infection and delayed wound healing. Therefore, surgeries using artificial materials in patients receiving tocilizumab require careful observation. A 75-year-old man who had been receiving tocilizumab and steroids for the treatment of adult Still's disease was diagnosed with an inguinal hernia and underwent laparoscopic transabdominal preperitoneal repair. Tocilizumab administration was discontinued for 3 weeks before surgery and was restarted 2 weeks after the surgery. Postoperatively, there was no fever, and the C-reactive protein level was marginally elevated. The patient was discharged from the hospital on the fourth day without any postoperative complications, and no delayed infection or delayed wound healing 1 year after the surgery. A few weeks withdrawal of tocilizumab administration before and after surgery permitted safe laparoscopic hernia surgery using a surgical mesh without infection or delayed wound healing.Coronavirus disease 2019 (COVID-19) was thought to have respiratory symptoms as the main manifestation, but it has become clear that extrapulmonary symptoms such as gastrointestinal disorders also occur. There are several reports of intussusception associated with COVID-19 in children, but these are rare in adults. In this report, we present a case of cystic intestinal duplication that enlarged during the course of COVID-19 treatment and resulted in intussusception. Right hemicolectomy was performed for intussusception due to the cystic lesion. To the best of our knowledge, this is the first resected case of intussusception due to alimentary tract duplication after COVID-19 infection. We hereby present a rare cause of acute scrotum secondary to emphysematous epididymo-orchitis (EEO). It is often not diagnosed until crepitus is palpable in the scrotal wall with spreading cellulitis, at which point it has reached an advanced stage. This case report details a 55-year-old man with poorly controlled diabetes who presented with an acute scrotum that failed to resolve with oral antibiotics in primary care. Following rapid detection of EEO on an early scrotal ultrasound scan, the patient had surgical debridement and a near-total orchidectomy with only a small stump of testis and tunica albuginea left behind. This case highlights the importance of timely imaging, particularly in diabetic men with an acute scrotum with a high clinical suspicion of intratesticular abscess. An incidental but significant finding of EEO would warrant early surgical intervention to prevent a catastrophic sequelae i.e. Fournier's gangrene.[This corrects the article DOI 10.1093/jscr/rjab198.].To date, there are no reports indicating the use of indocyanine green (ICG) fluorescence to detect pathologic lymphatic tissue when a laparoscopic lymph node biopsy (LLB) for suspected new or recurrent lymphoma is performed. We present the case of a 72-year-old female patient admitted for suspicion of recurrent lymphoma. A preoperative imaging work-up showed solid tissue enveloping the terminal portion of the abdominal aorta with a standardized uptake value (SUV) of 10. Therefore, an LLB was planned. After induction of anesthesia, a ICG solution was injected intravenously and subcutaneously at both inguinal regions. At laparoscopy, a complete visualization of the pathologic lymph nodes was achieved, enabling an incisional biopsy of the lymphomatous mass. LLB with ICG-fluorescence offers a simple and safe method for pathologic lymph node detection in the suspicion of intra-abdominal lymphoma. More studies with large case series are needed to confirm the efficacy of this application.Peutz-Jeghers syndrome (PJS) is an autosomal dominant mutation of the STK11/LKB1 gene on chromosome 19 often characterized by mucocutaneous pigmentation, hamartomatous polyps, anemia, gastrointestinal bleeding and intussusception. We present the case of a 21-year-old female with no pertinent family history who received the diagnosis of PJS after presenting to the hospital with two episodes intussusception. Patients with PJS have an increased lifetime risk of developing stomach, small bowel, colon, pancreatic, breast, cervical, uterus and testicular cancer requiring religious surveillance at an early age.
